The normal gallbladder is pear-shaped. It distinguishes the bottom, body and neck. The inflection usually occurs at the boundary between the body and the bottom of the bubble. It may turn out to be either a larger or a smaller section of it. Sometimes there are several folds that give the bubble the most diverse shape: boomerang, hourglass, Latin letter "S". There is a congenital inflection. This is not a disease, and a person, as a rule, does not experience any discomfort.

In contrast to congenital ultrasound, an inflection of the gallbladder is diagnosed due to pericholecystitis, which occurs with long-term chronic cholecystitis, cholelithiasis, after acute cholecystitis. When the inflammatory process passes to the outer wall of the gallbladder, the resulting adhesions deform it. In such cases, the contractile function of the gallbladder is disturbed, the qualitative and quantitative composition of bile changes and, as a result, digestion is disturbed. Clearly, those with acquired gallbladder kinking need treatment.

In some patients, sudden movements, straining when lifting weights cause a short-term incomplete twisting of the gallbladder around its longitudinal axis. It proceeds almost asymptomatically and passes without a trace. A similar condition can occur in the elderly with a pronounced prolapse of the internal organs, in the case of elongation of the neck and sagging of the bladder, with an increase in the gallbladder and the presence of stones in it.

Extremely rare cases when the gallbladder twists around its longitudinal axis several times. Such torsion, or inversion, entails a violation of blood circulation in the wall of the bladder and even necrosis (necrosis) of the wall. As a result, its perforation occurs, bile flows into the abdominal cavity, which is accompanied by a sharp pain in the right side of the abdomen, weakness, sweating, vomiting, bloating. Options for the shape of the gallbladder

Visualization of the bile duct during an echographic examination allows you to determine various options for its shape: kinks, constrictions, septa. To accurately determine the type of anomaly in the shape of the gallbladder, it is necessary to evaluate it in various planes, since on some sections the pattern of inflection, especially in the area of ​​the body and bottom, may be similar to that in the presence of a septum. In doubtful cases, when obtaining a picture of the partition, the sensor is turned by about 90 ° or oscillatory movements are made with it, changing the beam entry angle. Such a maneuver helps to change the scanning plane and see the inflection. If the picture has not changed, then there is a partition, and when the contour changes, there is a constriction. The septum appears as a strong linear echo going from one wall of the gallbladder to the other (complete septum) or protruding into the lumen (incomplete). The contour of the gallbladder does not change. In the presence of narrowing in the area of ​​the septum, we can talk about constriction, when the gallbladder takes on the shape of an hourglass.

An incomplete septum in the gallbladder should be differentiated from a polyp, which has the same echographic features: it protrudes into the lumen of the gallbladder, its echogenicity is the same as the wall, it is fixed to it. However, it is very difficult to distinguish these structures sonographically. One can think of a cholesterol polyp if a distal shadow is recorded from it. Usually it is not difficult to determine the inflection of the gallbladder in the body and neck, its S-shaped deformation. It is more difficult to see the inflection in the fundus, since this section is often covered by the transverse colon and is visible, especially in children with an underdeveloped subcutaneous fat layer, immediately below it.

Often the excesses of the gallbladder are inconsistent and disappear when the patient is examined while standing or with an inflated abdomen. On the other hand, quite often these maneuvers, on the contrary, lead to excesses of the bubble. An analysis of the causes of abdominal syndrome showed that gallbladder shape disorders are the most common cause of abdominal pain and organ motility disorders. An assessment of the frequency of anomalies in the shape of the gallbladder showed that the most common kink in the neck of the gallbladder (45%). However, it must be remembered that the gallbladder has a natural inflection in the neck when it passes into the cystic duct. In this regard, the ascertainment of a change in shape in this section is valid at an inflection angle of more than 90°. In 7% of cases, an inflection occurs in the bottom area, in 32% - in the body area, in the remaining cases, various combined inflections are noted.

The most common localization of the partitions is the cervical region (77%), less often they are noted in the body (12%) and in the bottom (1%). In some cases, there is a combination of inflection with partitions.

The gallbladder in newborns is spindle-shaped, and in subsequent years it usually takes on a pear-shaped or funnel-shaped form. It is attached to the lower surface of the liver. The bottom of the gallbladder is located in front, the body gradually passes into a narrow neck, turned backwards and upwards, from which the cystic duct departs.

At the point of transition of the neck into the cystic duct, the muscular layer of the wall thickens and forms an annular layer of muscles - the sphincter (pulp) of Lutkens. The internal, mucous membrane of the gallbladder is rich in folds and forms slit-like intrusions into the middle, muscular layer (Rokitansky-Ashoff passages), and in the initial section of the cystic duct it forms several spiral folds that can close a significant part of its lumen - Geister valves. Usually, the HP looks like it is shown in the figure (see above).

On an empty stomach, the gallbladder contains 30-80 ml of bile, but it can concentrate 5-10 times more hepatic bile. With stagnation of bile in the gallbladder, its amount in it may increase. In women, the gallbladder in a state of functional rest has a slightly larger volume than in men, but it contracts faster. With age, the contractile function of the gallbladder decreases.

In the human body, on average, 500-700 ml of bile is formed per day.

Unlike other organs of the digestive system (stomach, pancreas), the formation of bile in the liver is a continuous process. However, its entry into the intestine normally occurs mainly in the process of digestion. This is ensured by the reservoir function of the gallbladder and its rhythmic contractions with the consequent relaxation of the mentioned sphincter of Lutkens and then the sphincter of Oddi, located at the confluence of the common bile duct into the intestine. Synchronicity disorders in the work of the gallbladder and sphincter apparatus underlie biliary dyskinesias (biliary tract dysfunctions), which can cause episodic, recurrent pain in the upper abdomen. The proposed mechanism of biliary pain is spasms leading to disruption of bile flow and expansion (bursting) of the overlying parts of the system.

It is a paradoxical fact that violations of the emptying of the gallbladder occur in many people with cholesterol stones, but most of them (80%) never develop these pain symptoms.

With each meal, the BP decreases. Emptying is carried out by several alternating phases of contractions and relaxations and occurs within 15-25 minutes, depending on the volume and quality of food, and other factors. Foods high in fat can help reduce gallbladder volume by 80%. Bile enters the intestines, where it is involved in digestion. Food initiates about 75% of emptying of the gallbladder, approximately 25% of emptying occurs during fasting, periodically every 100-120 minutes.

A delay in the release of bile can occur as a result of blockage of the ducts by inflammatory products, a stone, or due to compression by a tumor (in these cases, the gallbladder can reach a huge size), as well as functional disorders in the form of a violation of the contraction of the muscles of the gallbladder and other parts of the biliary system. Motility of the biliary tract is often impaired due to dependence on a variety of reflex influences and the complexity of regulatory mechanisms.

The official classification of diseases specifically provides for only one congenital anomaly (malformation) of the gallbladder, namely its underdevelopment or absence at all (agenesis, aplasia and hypoplasia of the gallbladder). In practice, there are often other features of the anatomical structure of the gallbladder, which pediatricians often interpret as malformations.

Doubled gallbladder and separated by a septum (septated) gallbladder. Partitions consist of fibromuscular tissue, they can be single and multiple, more often located across the bladder, less often longitudinally, they can divide the gallbladder into separate cavities.

The ultrasound method of research (ultrasound) has become a kind of "gold standard" for assessing the condition of the abdominal organs. With the help of ultrasound, the time of diagnostic search is significantly reduced. The physical harmlessness of the study has led to the fact that it is often prescribed for unclear indications. Ultrasound often reveals deformation of the gallbladder and thickening of its walls, while computed tomography (CT), as a rule, does not detect this pathology (!). Unlike CT and magnetic resonance imaging (MRI), with ultrasound, the researcher himself is directly involved in obtaining the image, which has its pros and cons. Positive is the possibility of a more targeted and detailed study of the object under study. The negative side is that the quality of the image and its interpretation largely depend on the experience of the researcher and the correctness of the methods used by him. As they write in the specialized literature, ultrasound diagnosis of movement disorders of the gallbladder is one of the most difficult issues in clinical pediatrics and children's practical health care. Science has not yet provided a standardized model for diagnosing dyskinesias and permanent and transient deformities of the gallbladder. Let me remind you that the bladder almost constantly changes shape, in addition, the mucous membrane forms Rokitansky-Ashoff passages in its walls.

Deformities of the GB in children are found very often: in about 30% of children with abdominal pain, and in some - 10-12% - combined. Contrary to unsubstantiated claims, serious studies show that abdominal pain due to biliary dyskinesias equally often occur in children with GB deformities and in children with GB of a typical type 1 .

The message about the detection of kinks, bends and constrictions of the gallbladder in a child always worries parents. They are especially frightened when long-term medical supervision and treatment associated with dietary restrictions and even medications are recommended. Moreover, in some cases, these "defects" are discovered by chance in children who do not complain about digestion and are even generally healthy.
