    The active substance of Monural® - fosfomycin trometamol - is a broad-spectrum antibiotic, a derivative of phosphonic acid.
    Monural® has a bactericidal effect. The mechanism of action is associated with the suppression of the first stage of bacterial cell wall synthesis. Being a structural analogue of phosphoenol pyruvate, it enters into a competitive interaction with the enzyme N-acetyl-glucosamino-3-o-enolpyruvil transferase, resulting in specific, selective and irreversible inhibition of this enzyme, which ensures the absence of cross-resistance with other classes of antibiotics and the possibility synergism with other antibiotics (in vitro synergy with amoxicillin, cephalexin, pipemidic acid has been noted). The antibacterial spectrum of action of fosfomycin trometamol in vitro includes most of the usual gram-positive (Enterococcus spp., Enterococcus faecalis, Staphylococcus aureus, Staphylococcus saprophyt., Staphylococcus spp.) and gram-negative (E. coli, Citrobacter spp., Enterobacter spp., Klebsiella spp., Klebsiella pneumoniae, Morganella morganii, Proteus mirabilis, Pseudomonas spp., Serratia spp.) pathogens. In vitro, fosfomycin trometamol reduces the adhesion of a number of bacteria to the epithelium of the urinary tract.

    Pharmacokinetics
    Absorption: Monural® is rapidly absorbed from the gastrointestinal tract when taken orally.
    In the body it dissociates into fosfomycin and trometamol. The latter does not have antibacterial properties. Bioavailability after a single oral dose of 3 g ranges from 34 to 65%. The maximum plasma concentration is observed 2-2.5 hours after oral administration and is 22-32 mg/l. The half-life from plasma is 4 hours.
    Distribution: Monural® does not bind to plasma proteins, is not metabolized, and predominantly accumulates in the urine. When a single dose of 3 g is taken orally, a high concentration is achieved in urine (from 1053 to 4415 mg/l), 99% bactericidal for most common pathogens of urinary tract infections. The minimum inhibitory concentration of Monural® for these pathogens is 128 mg/l. It is maintained in urine for 24-48 hours, which requires a single-dose course of treatment. Concentrations of Monural® in urine above the minimum inhibitory concentration for E. coli persist for at least 80 hours.
    Excretion: Monural® is 90% excreted unchanged by the kidneys, creating high concentrations in the urine. About 10% of the dose taken is excreted unchanged through the intestines.     Use during pregnancy and lactation
    During pregnancy, the drug is prescribed only when the potential benefit to the mother outweighs the potential risk to the fetus.

This drug, when used quite often, can cause a number of complications. The most common disorders are the patient's digestive system.

Quite often, a Latvian-made drug, Furamag, is used in outpatient or inpatient practice. This medicine consists of furagin, a substance from the nitrofuran series. Additionally, it contains magnesium carbonate, which changes the pharmacological properties of the main active ingredient.

Thanks to it, furagin molecules are not broken down, and its bioavailability increases three times.

The drug is practically not metabolized. In the blood and peripheral tissues of the body (with the exception of lymphatic) its concentration remains low. But furagin is quickly excreted in large quantities by the kidneys, which allows it to be used for the complex treatment of bacterial pathologies of the urinary tract. Furamag showed good effectiveness against staphylococci, enterobacter, proteus, cytobacter and other pathogens.

The drug is released in the form of capsules of 50 mg. Usually it must be taken before meals, 1-2 capsules (50-100 mg) 2-3 times a day, with a sufficient amount of water. The course of treatment usually lasts at least 7 days.

Side effects that may occur when using Furamag usually include digestive tract disorders, toxic effects on the liver (cases of toxic hepatitis have been described) and the central nervous system (dizziness, headaches, tinnitus).

Another synthetic antibacterial drug that is used as an analogue of Monural is Furadonin. Its active ingredient is nitrofurantoin. This drug belongs to the nitrofuran group. Furadonin in experiments showed a predominantly bacteriostatic effect, which it realizes by inhibiting the synthesis of RNA and protein in the cells of pathogenic microorganisms.

The drug is well absorbed when administered orally in the digestive system. Furadonin is completely excreted from the body by the kidneys and is a classic uroseptic, since the range of its use is limited precisely by the pathologies of one system.

Nitrofurantoin effectively acts on staphylococcal, streptococcal, enterobacter microflora.

The drug is available in the form of tablets of 50 or 100 mg. The duration of therapy is usually from 5 to 14 days. An adult should take 1 tablet of the appropriate dose 3-4 times per day. Take with food to reduce possible side effects.

Furadonin should not be prescribed in the following situations:

  • if you have had allergic reactions to nitrofurans in the past;
  • with significant impairment of renal function;
  • during lactation;
  • during pregnancy;

  • patients with congenital or acquired deficiency of glucose-6-dephosphate hydradenase.

Among the side effects, the most common are transient disorders of the digestive tract (diarrhea, constipation, nausea, flatulence), which quickly disappear after discontinuation of the drug. The development of allergic reactions of varying severity was also noted. In some patients, exacerbation of systemic inflammatory diseases (lupus, vasculitis) was observed while taking the drug.
