The drugs of this group inhibit sodium reabsorption by 10-20%, therefore they are powerful, short-acting diuretics. Pharmacological effect both drugs are almost the same. The mechanism of action of furosemide is associated with the fact that it significantly increases renal blood flow (by increasing the synthesis of prostaglandins in the kidneys). At the same time, the inhibition of kaliuresis appears immediately after the administration of the drug. The activity of spironolactone is independent of the acid-base state. The drug has a significant duration of action (up to several days). This is a slow but long-acting. The drug increases calciuresis, has a direct positive inotropic effect on the heart muscle.

The drug inhibits the enzyme carbonic anhydrase, which normally contributes to the combination of carbon dioxide and water in nephrocytes with the formation of carbonic acid.
