Inflammation of the appendages - symptoms and treatment. From a conversation with Doctor of Medical Sciences A.F. Puchner
Inflammation of the uterine appendages- This is a very common disease. Every second woman experiences this disease in her life. Another name for the disease is adnexitis.

Causes of inflammation of the appendages

1. Hypothermia
2. Various infections
3. Weakened immunity.
It is especially dangerous when these three causes of adnexitis are present together. And this is not so difficult to organize. Stress, overwork, poor nutrition reduce immunity.
Infection can also often be present in the body in the form of inflamed maxillary sinus, purulent tonsils, carious teeth etc. And for hypothermia, it is enough to just sit on a stone parapet; after swimming in a pond, do not immediately change into dry clothes or put on a short jacket in cold weather.

Acute adnexitis - symptoms

1. Aching lower back
2. Pulls in the lower abdomen
3. It hurts to step on your foot.
4. Temperature rises

The course of the disease and the consequences of inflammation of the appendages

In a woman diagnosed with “adnexitis,” the fallopian tube becomes inflamed, it becomes painful and swollen. As a result of the disease, many adhesions are formed, which often deform the fallopian tubes. Which in turn leads to infertility or ectopic pregnancies.
How does inflammation of the appendages begin? The temperature rises, chills and sweating begin, aching pain. You should immediately consult a gynecologist.
What should a woman do while waiting for treatment if her appendages hurt? Under no circumstances should you use a heating pad, despite the chill. Cold should be applied to the suprapubic area. The pain will soon subside. As soon as you have strength, immediately see a doctor.

Treatment of inflammation of the appendages with medication

Treatment of adnexitis proceeds in several directions at once:
1. First of all, antibiotic treatment is prescribed intravenously or intramuscularly. Gentamicin, kanamycin, and others are used modern antibiotics, which are selected strictly individually.
2. Immunomodulatory agents: decaris, neovir, thymogen, viceron, echinacea, galavit.
3. Complex of vitamins and antiallergic drugs.

Success in the treatment of adnexitis depends on the patient’s mood. A woman should be avoided stressful situations, drive away everything negative, rejoice and smile at every new day.
A woman with a strong nervous system and a positive attitude can cure inflammation of the appendages much faster

Inflammatory diseases pelvic organs is one of the most common pathologies in the gynecological practice of doctors. According to medical indicators, about 60% of diseases of the female genital area develop against the background of inflammatory processes in the ovaries, uterus, fallopian tubes, and appendages.

The term "appendages" means everything adjacent organs uterus (fallopian tubes, ovaries). Inflammation of the appendages belongs to the group of infectious and inflammatory processes that most often involve the ovaries or tubes of the uterus itself. In cases where inflammation affects the uterus, symptoms of endometritis most often appear.

In gynecology, diseases of the appendages can be found under the name adnexitis (inflammation of the ovaries) or salpingoophoritis (inflammation of the fallopian tubes). Considering that these organs are anatomically closely related to each other, the doctor often makes a diagnosis of “inflammation of the appendages.” What are the causes and symptoms of the disease, how to treat inflammation of the appendages and how dangerous this disease is.

Inflammation of the appendages: causes

Normally, the pelvic organs, including the appendages, do not have pathogenic flora, but under certain factors or diseases, pathogenic bacteria are able to penetrate inside, provoking the development of inflammatory processes. It is known that any inflammatory disease is caused by pathogenic microorganisms. In this case, the cause of inflammation in the appendages can be any bacteria that has penetrated the mucous membranes genitourinary system: viruses, fungi, gonococci, chlamydia, streptococci, staphylococci, spirochetes, protozoa, herpes simplex viruses and others, which, after entering the body, actively multiply, causing symptoms characteristic of inflammation of the appendages. Any of pathogenic microorganisms can not only cause symptoms of inflammation of the appendages, but also other diseases. For example, chlamydia - chlamydia, fungi - vulvitis, candidiasis, gonococci - gonorrhea and other diseases that are often sexually transmitted.

Pathogenic microbes can enter the body not only through sexual contact, but also through contact, household contact, and also through failure to comply with basic hygiene rules or through contact with a carrier of the pathogen. Of particular importance in the development of this disease is the condition immune system. If a woman’s immunity is strong, it will not allow the activation of any microorganism. In cases where the immune system is weak, the risk of getting sick increases several times. In addition, there are a number of certain factors that are a trigger for the development of inflammatory processes in the appendages. Among these factors are:

  • Hypothermia of the body.
  • Overwork;
  • Constant stress;
  • Unbalanced diet;
  • Constipation;
  • Promiscuous sexual intercourse.
  • Abortion.
  • Childbirth;
  • Sexual intercourse during menstruation.
  • Poorly performed gynecological examinations.

In addition to the above factors, inflammatory processes in the appendages can be triggered by physical or mental stress, endocrine disorders, internal infections of other organs. In any case, inflammation of the appendages must be treated, since the absence timely treatment entails complex and sometimes dangerous complications.

Symptoms of inflammatory processes in the appendages

Inflammation of the appendages - symptoms can occur in acute or chronic form. The acute form of the disease has a pronounced clinical picture, and chronic inflammation of the appendages has more subtle symptoms, which are characterized by periods of remission and exacerbation. With inflammatory processes in the appendages, a woman experiences the following symptoms:

  • Pain syndrome. Pain is localized due to inflammation of the appendages in the lower abdomen. It can have different intensity and radiate to the sacrum or thigh. Pain increases during menstruation or sexual intercourse. If a woman suffers from a chronic form of the disease, then pain syndrome may be present constantly, intensifying with exacerbation.

  • Menstrual irregularities. Menstruation with inflammation of the appendages is almost always irregular, quite painful, and can be observed heavy bleeding. IN in rare cases Menses too short and scanty.
  • Vaginal discharge ranges from purulent to mucous with an unpleasant odor.
  • Itching, burning in the vaginal area.
  • Increased body temperature. During an exacerbation, body temperature can rise to 39 degrees.

  • General malaise. Occasionally, nausea, dry mouth, and increased fatigue are felt.
  • Disorders of the urinary system. Discomfort, pain when urinating.
  • Irregularities at work nervous system: increased irritability, depression.
  • Decreased libido.

Signs of inflammation of the appendages can be determined by the results of a blood test. During inflammatory processes, the blood formula changes significantly, and the ESR increases. In addition, when gynecological examination at an appointment with a gynecologist, a woman feels severe pain in the area of the ovaries and uterus. The above symptoms may also be present in other diseases of the genital organs, so put accurate diagnosis only a doctor can do so after examining the patient, collecting anamnesis, and laboratory and instrumental studies: blood test, urine test, ultrasound of the pelvic organs and others, which will allow the doctor to draw up a complete picture of the disease and make the correct diagnosis.

Possible complications

Inflammation of the appendages - symptoms and treatment should be carried out in a timely manner and only under the supervision of a doctor. In cases of untimely or incorrect treatment, there is a risk of developing complications that are not life-threatening for the woman, but can lead to the development of a chronic form of the disease, as well as infertility.

It is important to note that women who have had inflammation of the appendages are 10 times more likely to be diagnosed with an ectopic pregnancy. Complications develop due to the formation of scar tissue on the ovaries or tubes. In rare cases, the complication manifests itself as purulent processes in the fallopian tubes. This complication requires surgical removal of the fallopian tubes or ovaries.

Inflammation of the appendages: treatment methods

Every woman should clearly know how to treat inflammation of the appendages, but in any case, therapeutic measures must be carried out under the supervision of a physician. IN acute period illness, treatment is carried out in a hospital and should include both drug treatment, as well as diet and the right image life. In the acute period, a woman is recommended bed rest, eating low-fat and unsalted foods.

An important place in treatment is antibacterial therapy, the action of which is aimed at destroying the causative agent of the disease. The doctor usually prescribes antibiotics wide range actions, among which are the following drugs:

  • Penicillin group – Amoxiclav, Augmentin;
  • Cephalosporins – Ceftriaxone, Cefazolin;
  • Macrolides – Erythromycin, Macropen, Fromilid.

The doctor may prescribe other antibiotics that affect pathogenic flora. Antibiotics for inflammation of the appendages are prescribed in the form of injections for intramuscular or intravenous administration or tablet form for oral administration. In the acute period, it is recommended to take antibiotic injections, as they can quickly relieve the inflammatory process, thereby reducing the symptoms of the disease. The effectiveness of treatment should be observed on the first day. The course of treatment takes from 5 to 10 days. If no positive dynamics are observed, the doctor may change the antibiotic or prescribe more radical methods of treatment (surgery).

In addition to antibiotics, the doctor prescribes other medications:

  • Anti-inflammatory non-steroidal drugs: Ibuprofen, Intomethacin.
  • Vaginal suppositories. They have a positive effect on pathogenic microorganisms and act directly at the site of inflammation. These medications are administered before bedtime. They have antimicrobial, anti-inflammatory, bactericidal effects: Terzhinan, Polygynax, Klion-D and others. Such drugs do not affect the vaginal microflora, have a minimal number of contraindications and are well tolerated by the female body.
  • Vitamin therapy. Vitamins B, C and E are prescribed, as well as immunostimulants to improve immunity.

A good effect in treatment can be obtained from physiotherapeutic procedures: UV blood, electrophoresis, laser treatment, UHF and many other methods. Physiotherapy can be used both in the acute period and in the chronic form of the disease.

The uterine appendages - the fallopian tubes and ovaries - are located in the pelvis on either side of the uterus. Infectious agents enter the appendages from the vagina or uterus. Inflammation more often can be caused by staphylococcus, gonococcus, chlamydia. Separate inflammation of the tube and ovary is very rare; usually the inflammatory process involves the tube and ovaries.

Causes of inflammation of the appendages.

Oophoritis is an inflammation of the ovaries, often combined with inflammation of the fallopian tubes (salpingitis). The cause of the disease can be a variety of microorganisms; as a rule, they occur in combination and are often resistant to antibiotics. The inflammatory-changed fallopian tube fuses with the ovary, forming a single inflamed formation. The formation of pus in the fallopian tube leads to the destruction of ovarian tissue.

Symptoms of inflammation of the appendages.

Acute inflammation is manifested by pain in the lower abdomen and lumbar region, high temperature body, chills, impaired urination. IN chronic stage patients are concerned about pain in the lower abdomen, menstrual irregularities, discharge from the genital tract, and during exacerbations - deterioration in general condition. When pus breaks through into the abdominal cavity, a serious complication develops - inflammation of the peritoneum (pelvioperitonitis). Severe complication is the formation of adhesions in abdominal cavity, and, as a result, infertility.

Treatment of inflammation of the appendages.

Treatment is selected depending on the causative agent of the disease. In case of an acute process, rest, ice on the lower abdomen (in the first days), painkillers, antimicrobials(usually at least two). At the same time, restorative therapy and physiotherapy are prescribed. If drug therapy is unsuccessful, surgical treatment is performed.

Modern reliable methods for treating even chronic inflammation have been developed, including a course of antibiotic injections (gentamicin) directly into the ovaries, anti-inflammatory vaginal tampons and injections into the ovary of an enzyme (lidase) that resolves adhesions.

The appendages belong to the pelvic organs, they are located on both sides of the uterus and consist of two ovaries and fallopian tubes. The size of the ovaries depends on the age and physiological development of the woman’s body. On average, the size of an ovary is 2x3 cm. It is in the ovaries that female sex hormones are produced and eggs mature, which are then transported through the fallopian tubes to the uterus. The length of the fallopian tubes is about 10 cm, and the lumen size is from 2 to 4 mm. IN in good condition the uterine appendages are sterile. They do not contain pathogenic microflora.

When the state of the body, emotional, hormonal levels, the temperature regime changes and the condition of the appendages. Microorganisms can enter the ovaries and fallopian tubes from the vagina. Even the normal microflora of the body can become pathogenic under certain conditions.

Characteristics of adnexal pain

You can understand how the appendages hurt:

  • According to the location of the pain - the lower abdomen on both sides of the center, sometimes the pain radiates to the legs or back;
  • According to the nature of the pain - acute pain or dull pulling;
  • According to the time of exacerbation - 1-2 days before the start of menstruation and up to 1-2 days after its start, less often before the end;
  • In relation to sexual contacts- increased during sexual intercourse or immediately after it.

Pain in the appendages may be accompanied by increased temperature, decreased libido, menstrual irregularities, and changes in the nature of vaginal discharge.

When the appendages hurt, the symptoms may indicate the presence of an inflammatory process in the pelvic organs.

Forms of disease of the appendages

There are several forms of the disease:

  • Spicy. Characterized by sharp increase temperature, acute pain lower abdomen, pronounced other symptoms;
  • Chronic (sluggish). The symptoms are mild, the temperature rises slightly, the pain is nagging, aching;
  • Latent (hidden).

The disease acquires chronic forms when the acute form of the disease is not treated or not treated at all. Symptoms may include irritability, loss of appetite, nausea, fatigue. Considered the most dangerous hidden form disease, since without symptoms the disease can progress undetected for a very long time. The presence of the disease cannot be detected without laboratory tests and therefore cannot be treated.

It is after the latent course of the disease that the most serious complications arise. Inflammatory diseases of the appendages can lead to consequences such as infertility and ectopic pregnancy.

Causes of diseases of the appendages

Let's look at the causes of inflammation:

  • Sexually transmitted diseases: specific venereal diseases and diseases caused by conditionally pathogenic microflora (chlamydia, gonococci, staphylococci, mycoplasmas, ureaplasmas, etc.);
  • Hormonal imbalances leading to menstrual irregularities. As a result, even cysts can form;
  • Temperature effects: hypothermia and overheating. Any adverse effect on the body natural factors leading to weakened immunity;
  • Irregular sex life. Sexual contact leads to normalization of blood supply in the pelvic organs and removes congestion;
  • Viral diseases. Inflammation of the appendages can occur as a complication after a common ARVI or flu.

The appendages hurt during pregnancy for two reasons: due to physiological characteristics the body of a pregnant woman or due to inflammatory processes. In any case, an urgent medical examination is required and laboratory tests.

Outside the acute stage, pain due to inflammation of the appendages decreases and subside.

Actions for pain in the appendage area

Pain in the appendage area requires examination by a gynecologist. An examination by a female doctor at least twice a year is needed to determine the asymptomatic course of various diseases of the appendages and their timely treatment.

When the appendages hurt, what should you do? Do not hesitate, but go for a comprehensive examination with a gynecologist, strictly follow the prescribed treatment regimen, use prescribed medications and carry out all procedures recommended by the doctor. If possible, the examination can be carried out in two medical institutions.

Don't stop treatment whenever you want. Remember that the more advanced the process, the longer the course of treatment.

Inflammation of the uterine appendages is quite common gynecological disease. If you do not seek qualified help in a timely manner or start incorrect treatment, there is a high risk of developing serious complications. The disease can become chronic and even cause infertility.

    The essence of pathology

    Inflammation of the appendages, also known as adnexitis, is an inflammatory processThe main appendages of the uterus are the ovaries, fallopian tubes and ligaments.

    Most women suffering from this disease cannot have children.

    Many people don’t even know where the appendages are located in women. They are located between the end of the fallopian tube and the ovary. This part is especially susceptible various diseases, and most often it is inflammation of the appendages; the symptoms begin to bother the woman almost immediately.

    Pathology carries infectious nature. It occurs due to the proliferation of pathogenic microorganisms. Can occur in both chronic and acute form. Since the appendages have a paired structure, the disease can develop on one side or simultaneously on both sides.

    An infection can develop in a woman’s body for many reasons. It spreads through the blood vessels and lymphatic vessels, occurs during complicated childbirth, failure to comply with personal hygiene rules, hypothermia, stress, reduced immunity, etc.

    Characteristic signs

    Signs of inflammation of the appendages are quite easily confused with other pathologies, for example, with a tumor in the intestine or acute appendicitis.

    Many women think that the problem of their illness is renal pathology, and by starting self-medication, they can significantly worsen the situation.

    Because painful sensations can concentrate in the lumbar region, inflammation of the appendages is easily confused with rheumatism. It seems that the problem lies on the surface, while it is inside.

    Local manifestations

    Inflammation of the appendages significantly affects the condition of the body. The woman’s quality of life noticeably decreases, and unpleasant symptoms are observed. Local signs include:

    • Pain in the appendages, which is localized in the lower abdomen and can be of varying intensity. Most often, not sharp, but aching pain is felt, which gradually moves to the sacrum and thigh. In the chronic form of the disease, the pain becomes regular. This is felt especially strongly during menstruation, during intimacy and during intense physical activity.
    • Crashes in menstrual cycle. Now your periods will regularly start earlier or significantly later. At the same time, they will be painful, the discharge will be abundant and prolonged. Sometimes the opposite situation may be observed - the discharge is scanty and lasts less than usual.
    • Discharge. They can be mucous, serous, with the presence of pus. The process is often accompanied by genital irritation and itching.

    General symptoms

    General signs of inflammation of the appendages in a woman are as follows:

    • Increased body temperature. Most often, the patient suffers from chills, muscle pain and headaches. In some cases, the temperature can reach 30 degrees.
    • Malaise. The mouth feels dry, the woman may feel nauseous and vomit, while the urge to empty the stomach is not frequent, but simultaneous.
    • Problems with the gastrointestinal tract. You often feel bloated, sharp pains, loose stools.
    • Disturbances in the functioning of the genitourinary system. Pain and burning may occur during urination. At the same time, the urge is rare. Sometimes pyelonephritis and cystitis develop against the background of this disease.
    • Endocrine and nervous system disorders. The ability to work decreases, there is a loss of strength, possibly depressive state. Some patients complain of loss of sexual desire, and intimate intimacy itself becomes unpleasant and painful for the woman.
    • One of common symptoms leukocytosis can be detected in the blood. Gradually, the blood formula may change. If upon examination the patient exhibits painful sensations ovaries and uterus, a specialist can make a diagnosis without doubt.

    Acute form of the disease

    The acute form of the disease is provoked by gonococci, but can also occur due to the proliferation of other microorganisms. Pathological disease is developing rapidly. Body temperature rises almost immediately and chills appear. Sharp piercing pain occurs in the lower abdomen. It can radiate to the rectum, lower back and legs.

    At first, you can observe noticeable muscle tension, especially when it comes to abdominal wall. Along with the main symptoms of inflammation of the appendages in women, there are signs of intoxication, which manifest themselves in the form of headache, sudden loss of appetite, nausea and vomiting, which also requires symptomatic treatment. The size of the appendages increases.

    If proper treatment does not occur within the next 10 days, the disease can quickly become chronic. During this period, pain begins to subside and becomes more sluggish. After some time of remission, another exacerbation is observed.

    Chronic pathology

    The chronic stage of the disease begins if the inflammation of the appendages was not treated or the treatment was carried out illiterately, without qualified assistance. Clinical picture The disease is inaccurate, usually an exacerbation is observed in the autumn and spring.

    Signs of inflammation of the appendages in women, which are chronic, do not differ significantly from those listed above. As a rule, these are dull and aching pains. Most often they are felt in the lower abdomen, but can be localized in the lower back and radiate into the vagina. As with the acute form, unpleasant thick discharge with purulent impurities.

    During the period of remission, body temperature may rise slightly. The pain is insignificant, but with strong physical exertion, during menstruation and sexual intercourse, it can become quite intense. Along with this, a woman may experience itching. Menstruation is irregular.

    Directions of therapy

    Not many people have a clear idea of how to properly respond to a problem. Most people do not even suspect that behind the pain lies inflammation of the appendages in a woman; symptoms cannot always help identify the essence of the problem, and, accordingly, treatment will begin too late. If your lower abdomen hurts, this is the first warning sign.

    The most important thing is that treatment is timely. If a woman has severe pain in her appendages, treatment is carried out in a hospital. The patient is assigned special diet, it excludes the use of salt, spices and carbohydrates. In the first days, the patient should remain in bed and be at rest.

    At acute course diseases can be alleviated with the help of available means. To do this, you need to apply cold to the lower abdomen. This will not only relieve pain, but also reduce inflammation.

    Taking antibiotics

    Medicines play the most important role in treatment. The doctor prescribes the drug depending on the causes of the disease and its specificity. It must be effective and have a maximum half-life.

    Inflammation of the appendages can be caused by various microorganisms. Therefore, the patient can be prescribed two types of tablets at once:

    • Klaforan - used 2 times a day, in a dosage of no more than 1 g.
    • Gentamicin – 80 mg is taken 3 times a day.
    • Cephobid - can be used together with Gentamicin, 1 g 2 times a day is enough.
    • Cefazolin is used similarly to the previous drug.
    • Lincomecin is used 3 times a day, 0.6 g.

    If the doctor suspects aerobic infection, Metrogyl tablets are additionally prescribed. Experts also recommend carrying out detoxification measures - intravenous infusions. Solutions consist of salts, glucose and hemodesis.

    Symptoms of inflammation of the appendages or ovaries in the form of pain and burning are eliminated with non-steroidal anti-inflammatory drugs. They are needed to remove acute inflammation and relieve pain. These can be not only tablets, but also suppositories. The doctor himself will select the right medication for the patient; you should not experiment with your health on your own. Usually these are drugs such as Ibuklin, Nurofen, Diclofenac, Ibuprofen, etc.

    In addition, it is necessary to support the body with vitamin complexes. Particular emphasis is placed on vitamins C and B. If allergic reactions occur, a woman can take Cetrin and Suprastin - these are some of the most effective antihistamines.

    Physiotherapy

    To relieve symptoms and alleviate a woman’s condition during acute or chronic inflammation, physiotherapeutic treatment is actively used, for example, exposure to impulses and ultrasound.

    In addition, in the hospital, patients can be provided with medicinal injections, immunomodulators. In the chronic form of the disease, it is recommended to relax in sanatoriums, carry out mud treatments, douching and medicinal baths.

    Using candles

    Candles are actively used to relieve inflammation of the appendages, lower the temperature, relieve pain and reduce swelling. Additionally, suppositories can be used to strengthen the immune system. During the treatment period they can play a significant role. Except listed properties, candles perfectly cleanse a woman’s body of accumulated harmful substances.

    Suppositories with an anti-inflammatory effect usually include diclofenac and many other non-steroidal drugs. Most of them cannot be used long period time. This can cause significant harm to health.

    The doctor may also prescribe suppositories with absorbable properties. Their main component is terrilitin. This substance is capable of affecting necrotic tissue and breaking it down; it is the result of the vital activity of mold. The product remarkably thins blood clots and pus.

    Suppositories will not only help to quickly and effectively relieve inflammation, but will also strengthen the vaginal walls and increase immunity. The drug can lower the temperature and relieve unpleasant pain.

    Folk remedies

    Treatment of inflammation of the appendages can also be carried out using alternative medicine, however, it should be remembered that you cannot use folk remedies alone. Such treatment may not give the desired result. It is important that activities are carried out comprehensively. Various infusions and decoctions should overlap with the medicinal technique.

    Natural ingredients can be wonderful prophylactic, special attention they should be given in the chronic stage of the disease. Despite the recommendations, many women refuse to go to the doctor and get diagnosed, preferring to be treated at home.

    Traditional medicine in this case can be a good alternative, because the possibility of side effects is minimized. The only thing a woman can suffer from is allergic reaction on one or another natural component.

    Help from wormwood and warming up

    Ordinary wormwood can give a good result. To do this, you need to pick as much of the plant as possible. Then 2 liters of water are boiled in an iron bucket and wormwood is thrown into it. A woman needs to sit over a bucket and sit over the steam for about 20 minutes. The disease will soon recede, and the inflammation will begin to subside quickly if this procedure is repeated for a whole week.

    You can also use the dry heating method. You will need 3 kg of salt, which must be heated in a frying pan or other container to a temperature of 80 degrees. After this, the salt is carefully poured into a thick fabric bag.

    Dried hops are poured into another bag; you need to fill in about 3 cm of dry plant. A dry compress with salt is placed on the hip area. In this case, you need to be in a stomach-down position.

    You need to place the hops on the lumbar area and press them tightly. The most important thing is that the salt and hops are opposite each other. Additionally, the woman is covered with a warm towel; she should remain in this position for several hours. The procedure is repeated twice a day for a week. Then you should take a break for 2-3 days, and you can repeat the therapy again.
